Flu Shots Still Available At UHC

 

Flu shots can still be obtained by students, faculty and staff at the University Health Center. Please call for an appointment at (973) 655-4361.  The cost of the flu shot for 2008 will be $15.00. 

 

Flu shots can also be obtained from private healthcare providers and community clinics.  Call your local Health Department for medical clinic locations and dates.

 

Students do not need to prepay for the vaccine but may do so by following the instructions below for faculty and staff payment. 

 

Faculty and staff must pay prior to their appointment in the following manner:

 

1.         Print out two copies of the form below and complete.

2.         Bring both copies and payment to Student Accounts (Bursar).

3.         Bring payment receipt to scheduled appointment. Flu shots will not be       administered to faculty and staff without proof of payment.
