Academic Advising Alert Form

In an effort to close the communication loop regarding students in your courses about whom you might be concerned, CAST has created the following survey tool. This tool can be used at any point during the semester and will be routed to the student's advisor.  Please also include as much information as you can about the student as well as your contact information to allow for the student's advisor to follow up with you.

With one of the links below, you can alert your college's CAST liaison who will, in turn, forward your concern to that student's academic advisor. Simply identify the Liaison for the department in which you teach and click on their link below. You will be asked to fill out a brief survey of the areas about which you are concerned for the student and an email will be automatically sent to the CAST Liaison for your department.
