Upload Checklist to <a href=\"https:\/\/www.njccis.com\/njccis\/home\">NJCCIS<\/a> with required documentation<\/p>\n<p>3. Submit the <a href=\"https:\/\/www.signupgenius.com\/go\/GNJKRating?CFID=5924bdf4-3ecb-43af-b83e-5adc20b83d69&amp;CFTOKEN=0#\/\">Two-Month Observation Window Request Form<\/a> to request your preferred two-month observation window<\/p>\n<p>4.
