The following information should be provided:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Name of the Deceased<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Date of Death<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Name of the individual reporting the death and the relationship to the deceased<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">After receiving the information, HR Benefits will notify the State and reach out to the family to coordinate the transition of the benefits.\u00a0<\/span><\/p>\n<div class=\"prpl-drawer\"><div class=\"prpl-drawer-header\">Alternate Benefit Program &lpar;ABP&rpar;<\/div><div class=\"prpl-drawer-content\">\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Pre-Retirement Death Benefit<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Beneficiary receives total value of account(s) including employer contributions and all investment earnings<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Life Insurance<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">3.5X the employee\u2019s salary based on the 10-12 months immediately preceding the employee\u2019s death<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/div><\/div><div class=\"prpl-drawer\"><div class=\"prpl-drawer-header\">Public Employee&apos;s Retirement System &lpar;PERS&rpar;<\/div><div class=\"prpl-drawer-content\">\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Pre-Retirement Death Benefit<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Beneficiary receives the return of the employee\u2019s contributions with interest<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Life Insurance<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">3X the employee\u2019s salary based on the 10-12 months immediately preceding the employee\u2019s death<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/div><\/div><div class=\"prpl-drawer\"><div class=\"prpl-drawer-header\">Police and Fireman&apos;s Retirement System &lpar;PFRS&rpar;<\/div><div class=\"prpl-drawer-content\">\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Pre-Retirement Death Benefit<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">A monthly survivor\u2019s pension paid to a specific family member or return of contributions to a beneficiary<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Life Insurance<\/span>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">3.5X the employee\u2019s salary based on the 12 months immediately preceding the employee\u2019s death<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/div><\/div><div class=\"prpl-drawer\"><div class=\"prpl-drawer-header\">Supplemental Retirement Plans &lpar;403b&sol;457b&rpar;<\/div><div class=\"prpl-drawer-content\">\n<ul>\n<li><span style=\"font-weight: 400\">Beneficiary receives total value of account(s) and all investment earnings<\/span><\/li>\n<\/ul>\n<\/div><\/div>\n","protected":false},"excerpt":{"rendered":"<p>Upon the death of an employee the employee\u2019s family should notify their department or HR at 973-655-7736 of the death.
