Details on All Student Immunization Requirements

Full-time (Undergraduate – 12 or more credits. Graduate – more than 6 credits)
- MMR 2-dose series (first dose AFTER first birthday)
- HepB 3-dose series (2nd dose at least 4 weeks after 1st dose. 3rd dose at least 8 weeks after 2nd, PLUS at least 16 weeks between 1st and 3rd dose.
- Meningitis 2 doses (Menactra, Menveo or MenQuadFi) Please Note: One dose must be given on or after 16th birthday.
- COVID-19 (Residential Students): Both doses in a 2-dose series (Pfizer-BioNTech, Moderna) OR one dose of a single-dose vaccine (Johnson & Johnson Janssen) and booster dose is encouraged. Please Note: International residential students must present WHO-approved vaccination (2-dose series OR single dose vaccine). A booster dose is encouraged.
Part-time (Undergraduate – less than 12 credits. Graduate – less than 6 credits)
- MMR 2-dose series (first dose AFTER first birthday) Please Note: You may see individual doses of Measles, Mumps and Rubella vaccine.
- Meningitis 2 doses (Menactra, Menveo or MenQuadFi) Please Note: one dose must be given on or after 16th birthday.
- COVID-19 (Residential Students): Both doses in a 2-dose series (Pfizer-BioNTech, Moderna) OR one dose of a single-dose vaccine (Johnson & Johnson Janssen) and booster dose is encouraged. Please Note: International residential students must present WHO-approved vaccination (2-dose series OR single dose vaccine). A booster dose is encouraged.

Students Exempt from immunizations other than the COVID-19 vaccine series (or a single dose series):
- Exclusively enrolled in a Certificate program. Please Note: Students in combined degree and certificate programs must submit immunizations.
- Exclusively enrolled in an official Montclair State University Online Program.
- Students age 31 or older at time of matriculation.
- Please Note: All students (undergraduate, graduate, full-time, part-time) are required to complete the COVID-19 vaccine series (or a single dose series) unless they have been approved for a medical or religious exemption.
Please review the policy for students with COVID-19 exemptions before completing the exemption request.
- Medical Exemption: Medical Exemption Form. Completed by a licensed healthcare provider and submitted to the MyHealth Portal for review and approval.
- COVID-19 Vaccine Religious Exemption: COVID-19 vaccination is required to live in the residence halls. If you have questions please contact Residence Life at reslife@montclair.edu or the Dean of Students Office at deanofstudents@montclair.edu.
- Religious Exemption: Complete the Religious Exemption Form. Save a copy of the form to your desktop. Go to your MyHealth Portal and using your University login upload your religious exemption form.

Please Note: All exemption forms for new students starting in fall 2023 must be submitted by Monday, May 22.
Late Admissions: Students who are admitted after Monday, May 22 should submit their proof of vaccination or request an exemption no later than two weeks after their admission date.
- How to Upload Your Medical Exemption to MyHealth Portal (Medical only), go to Upload Documents and select the “Medical Exemption” tag to upload.
- You can then go to My Profile → Images to confirm your images have been uploaded properly.
