Financial Aid Forms

To Submit Financial Aid Documents

  • Aid Adjustment Form
    • Complete this form if you want to reduce, cancel or reinstate your aid. (You must have Google Apps Activated on your Montclair State University account)
  • Cost of Attendance Re-Evaluation Form
    • Complete this form if your actual school-related expenses are higher than expenses listed on NEST.
  • Consortium Agreement Form
  • Loan Reaffirmation Form
    • Complete this form if you are requested to complete a loan reaffirmation with your servicer, due to over-borrowing the allowable federal loan limits. Once complete, you need to send the form to your loan servicer that holds the over-borrowing loan (do not send this to Montclair State University, only the loan servicer can process this request). You will need to contact the school where the over-borrowing took place to complete section 3.
  • SAP Mid-Year Re-evaluation Request
  • Summer Loan Request
    • Complete this form if you are applying for a loan for your summer courses. (You must have Google Apps Activated on your Montclair State University account)

Please upload the completed forms into FinAid Student Forms via NEST or send via the instructions on the form.

Office of the Registrar Forms

The following processes for undergraduate students are completed through your NEST account. Request to Repeat a Course, Change of Major, Withdraw, Leave of Absence, Pass/Fail Request and Enrollment Verification. Undergraduate credit adjustments are initiated by your academic advisor.

Please Note: Some forms may be signed using your electronic signature. Wherever that function is not available you will need to print, sign, scan and upload your document. File Hawk all completed forms to recipient (NETID) Registrarforms. Any forms missing a required signature will not be processed.

Instructions for Sending Office of the Registrar PDF Fillable Forms via MSU File Hawk

  • Complete the appropriate PDF Fillable form.
  • Save the completed form to your computer as a PDF and then send it via MSU File Hawk, other file formats will not be accepted.
  • Do not “lock” or add a password to your form as we may need to edit the form.
  • The Registrar’s office will not accept forms that are sent as attachments from your email.
  • Important: When asked to enter “File Recipient(s) NetID:”, please enter: Registrarforms
  • Do Not Enter Your Own NetID as the “File Recipient”
